Who We Are
We are older adults, caregivers, providers, and aging specialists working together to improve health on Chicago’s South Side.
As a Geriatrics Workforce Enhancement Program (GWEP), we educate geriatrics and primary care providers and trainees in integrated age-friendly care, and partner with local organizations to address gaps in care for older adults, promote dementia-friendly communities, and address social determinants of health. GWEP is federally funded through the Health Resources and Services Administration (HRSA).
Why We Do It
Older adults on the South Side have unique health care needs and require a unique approach to care.
There are too few geriatrics specialists to meet the current and future needs of older South Side residents, and many providers and community members lack training to meet the health care needs of older adults.
How We Do It
SHARE Network’s multi-level approach to improving the health of older adult community members means our program touches many different people and professions. Impacting the health of our community requires training health providers, collaborating with community leaders, and connecting face-to-face with older adults and caregivers.
