Chicago Hyde Park Village is a membership based organization that fosters engagement between older adults and the surrounding Hyde Park neighborhood. With a focus on promoting personal agency among its members, CHPV offers volunteer assistance, access to vetted resources, and classes and activities that help connect older adults with the surrounding community.

Mather is a non-denominational not-for-profit organization dedicated to creating Ways to Age Well.^SM Founded in 1941, Mather provides unique senior living residences, inspiring community programs, and innovative research through Mather Institute. Mather’s community initiatives center creative engagement, brain health and lifelong learning through virtual and in-person programs specifically designed to meet the wellness goals of adults 55 and better. The vision of Community Initiatives at Mather is an equitable society in which all older adults find wellness opportunities and resources to thrive within their communities. From creative mindfulness, to yoga, to digital literacy and fiber arts, our programs focus on increasing meaningful social connections, encouraging choices and providing opportunities for personal growth. All programs are free and accessible online or through our community partners.

Symphony Care Network offers long term care and rehabilitation services at 28 locations throughout the Chicagoland area. With a dedicated staff of clinicians and caregivers, Symphony Care Network offers an innovative, proactive approach to patient wellness.

Oak Street Health is a network of primary care practices, many of which are located in medically underserved communities, providing value-based care specifically to adults on Medicare. OSH operates 15 clinics in the Greater Chicagoland Area, and over 55 clinics across a 6 state region.

The Renaissance Collaborative’ s Senior Village is an independent living facility dedicated to helping older adults age in place. Located in the Washington Park neighborhood, TRC provides residents with affordable housing, recreational activities, continuing education resources, and workforce development.

The SHARE Network is housed within the University of Chicago Medicine, Biological Sciences Division, Section of Geriatrics. Being part of a world-class academic medical institution provides us opportunities to collaborate with knowledgeable faculty in our shared mission of promoting health equity and improved outcomes on the South Side of Chicago.
