Mile Square Health Center - Back of the Yards

Part of the University of Illinois Hospital & Health Sciences System, this Federally Qualified Health Center (FQHC) provides primary care, urgent care, and other services such as referrals for housing and substance abuse programs. Fees are on a sliding scale for the uninsured.  Walk-ins are welcome or call  (312) 996-2000 to make an appointment.
