Heartland Health Outreach Oral Health Services

Heartland Health Outreach Oral Health Services provides dental check-ups, cleanings, fillings, limited extractions and x-rays. Appointments are required for service, and fees are calculated according to a sliding scale based on income, with Medicaid accepted. To be accepted for service, individuals must have photo identification and social security card (if eligible). If employed, proof of income or other income is required. If person has insurance, relevant insurance information (Medicaid card, Medicare card, private insurance card) is required. People who are experiencing homelessness must have a medical visit with a primary care provider at Heartland Health Center – Uptown. People who are living with HIV/AIDS must have a medical visit with a primary care provider (this may be a Heartland Health Center – Uptown Provider, or it may be a provider from another health center), laboratory test (CD4, viral load) updated at least twice yearly and a referral updated at least annually from a primary care provider.
