Chicago Hyde Park Village (CHPV)

Chicago Hyde Park Village is a membership based organization that fosters engagement between older adults and the surrounding Hyde Park neighborhood. With a focus on promoting personal agency among its members, CHPV offers volunteer assistance, access to vetted resources, and classes and activities that help connect older adults with the surrounding community.


Mather meets the unique needs of older adults through a wide range of programs including senior residence buildings, nutritional education, and recreational activities. With an overarching goal of helping seniors age well, Mather affords individuals the opportunity to maintain a sense of autonomy as they age. Mather’s—More Than a Café are more than simply cafe, they offer live entertainment, fitness classes, day trips, and lifelong learning opportunities. From watercolor workshops to lectures on Chicago history to Latin dance classes, there’s something for everyone at Mather’s—More Than a Café. There’s no membership—everyone is welcome to come in, grab a bite or a bottomless cup of coffee, and check out what’s happening that day.

Symphony Care Network

Symphony Care Network offers long term care and rehabilitation services at 28 locations throughout the Chicagoland area. With a dedicated staff of clinicians and caregivers, Symphony Care Network offers an innovative, proactive approach to patient wellness.

Oak Street Health

Oak Street Health is a network of primary care practices, many of which are located in medically underserved communities, providing value-based care specifically to adults on Medicare. OSH operates 15 clinics in the Greater Chicagoland Area, and over 55 clinics across a 6 state region.

The Renaissance Collaborative (TRC) – Senior Village

The Renaissance Collaborative’ s Senior Village is an independent living facility dedicated to helping older adults age in place. Located in the Washington Park neighborhood, TRC provides residents with affordable housing, recreational activities, continuing education resources, and workforce development.

University of Chicago Medicine

The SHARE Network is housed within the University of Chicago Medicine, Biological Sciences Division, Section of Geriatrics. Being part of a world-class academic medical institution provides us opportunities to collaborate with knowledgeable faculty in our shared mission of promoting health equity and improved outcomes on the South Side of Chicago.
