Chicago Family Health Center

Chicago Family Health Center offers a range of dental care services, including specialty care for children and pregnant women at a sliding scale fee. Services include cleanings, exams, x-rays, dental sealants, restorative treatments, and extractions. Call to make an appointment at the facility nearest to you. To complete registration, must bring photo identification, your current address and phone number, and medical or insurance card (if you have one). If you don’t have a medical card or insurance, bring income information, such as two paycheck stubs, W-2 form, Medicaid card, food stamp card, or unemployment notice.
