The University of Chicago Medicine Outpatient Senior Health Center offers medical care for adults age 65 and older, including: Alzheimer's Care Giver Support Group, Chronic pain assessment and treatment, Comprehensive geriatric assessment, EKG and laboratory testing, Frailty evaluation, Geriatric psychiatry, Joint injections, Memory assessment, Nutrition counseling, Primary care, Social services and Urgent care.

Since 1936, Salvation Army's Family and Community Services (FCS) has been providing direct services to low income and vulnerable seniors (ages 60 and older) and is currently the largest provider of non-residential senior services in the City of Chicago. Well-being checks are provided by FCS 7 days a week through the 311 hotline. Intensive case advocacy and support is provided to surround the senior in crisis with resources to move to stabilization. Additionally, housing assistance is provided to help seniors facing a housing crisis find safe, decent, and affordable housing in Chicago. Senior caregiver support is provided in the form of financial assistance, individual and group counseling, assessments and respite care to caregivers of seniors 60+. Older Relatives Raising Children provides financial assistance, counseling, support groups, respite, & case management services to older relatives 55+ raising a relative child, including community outreach.

The Alzheimer's Association 24/7 Helpline provides reliable information and support to those who need assistance. Calls are toll-free anytime day or night at 1-800-272-3900. The 24/7 Helpline serves people with memory loss, caregivers, health care professionals and the public. Trained and knowledgeable staff can help you with understanding memory loss, dementia and Alzheimer's, medications and other treatment options, general information about aging and brain health, skills to provide quality care and to find the best care from professionals, and legal, financial and living-arrangement decisions. The helpline also features confidential care consultation provided by master's level clinicians who can help with decision-making support, crisis assistance and referrals to local community programs, services and ongoing support. Additionally, help in a caller's preferred language is available.

Renewal Care Partners provides licensed healthcare and social services to older adults and people with chronic health conditions in their places of residence. In Chicago, Renewal Care Partners offers companion care and social programs to those 60 or above seeking intellectual stimulation and support through early to middle stages of memory loss. In addition to in-home services, Renewal Care Partners offers Memory Cafes, free monthly social gatherings for persons living with dementia and their family, friends, and care partners. Renewal Care Partners accepts private pay and some forms of long-term insurance, but does not accept Medicaid/Medicare.

Trinity United Church of Christ offers open caregiver support groups for family members taking care of a loved ones.

Meeting Times:
1st Wednesdays from 12:30pm to 2:30pm
4th Tuesdays from 6:00pm to 8:00pm.

The JCC Chicago together with Northwestern Movement Disorder Center is now offering Parkinson's Disease programming at Hyde Park JCC.  A monthly membership includes weekly exercise classes and a monthly support group.  Education events are also offered throughout the year.  Financial assistance is available.  For more information contact Emily Zivin ( emily.zivin@northwestern.edu, or call the number below). We provide Adult Protective Services though assessments, referrals for in home services, assist with applying for health insurance and advocacy & protection for vulnerable adults.   

Additionally we provide Counseling & Group Support to Caregivers who take care of adults 60 and older or adults 60 and under with Dementia, Alzheimer's Disease, TBI or any kind of neurological disorder.

Cancer Support Team, Inc is a non-profit organization with the purpose to educate, coordinate, and promote the support of cancer patients and their families on a local level.  Cancer Support Team,Inc. helps guide individuals on how to maintain a spiritually healthy lifestyle by connecting with and assisting survivors and caregivers on their journey in living with a cancer diagnosis. Some of the services offered include inspirational literature, comfort bags, individual cancer patient support, resource assistance, and wigs.

The Caregiver Initiative offers family caregivers of adults 65 and older the education and support needed to take care of themselves while caring for others. It provides a thorough assessment followed by services tailored to the needs of family caregivers.

CASL offers various dementia caregiving support options including dementia caregiving support options including counseling, educational workshops, consultations, Savvy Caregiver Express training, and more.

The Mesulam Center for Cognitive Neurology and Alzheimer's Disease offers three monthly support groups for care partners of persons living the following forms of dementia: 1) Frontotemporal degeneration (FTD), 2) Primary Progressive Aphasia (PPA), and 3) Younger-Onset Dementia. The Buddy Program is an opportunity for persons living with dementia and their families to connect with and mentor a first-year medical student.
