Barton Senior Residences of Chicago offers seniors, 65 years and older, a supportive living option that combines the comfort and security of a community setting with the privacy of their own apartment. Residents must be mobile. 

Coles Supportive Living was developed to provide lower-income seniors and seniors with disabilities as an alternative to nursing home care. Depending on the individuals financial position and health status, they may qualify Medicaid will reimburse Coles Supportive Living for the cost of Assisted Living care. The program combines apartment style living with personal care and other services. The person can live independently; take part in his or her own decisions while receiving the personal care and attention required. Services for residents include: fitness classes, adult learning, nutrition programs, social activities, transportation, and geriatric medical care.

Jackson Park Supportive Living is an assisted living facility. Jackson Park strives to promote dignity and independence for seniors where a state-of-the art environment exists with safe and comfortable surroundings. The Facility strives to meet the needs of its residents by providing quality services and support. Services available to residents include fitness classes and nutritional programs.

The mission of Montgomery Place is to enhance the quality of life for a diverse and inclusive urban community of older adults, supporting each person’s physical, social, intellectual and spiritual needs. It offers independent and assisted living options, memory support, skilled nursing, and health and arts programming.

Pioneer Gardens Supportive Living meets the need for affordable housing and support services for seniors. Residents receive three meals a day plus snacks, weekly housekeeping and laundry, social, and educational activities. Residents also enjoy the convenience of various on-site amenities such as beauty/barber salon, community store and physical therapy center. Supportive Living is the alternative for the nursing home setting. All residents must be able to participate in their care and may not have health needs that require 24-hour skilled nursing care.

Avantara Park is a nursing and rehab venue in Chicago's South Suburbs. Clinical liaisons meet with patients and families at the hospital before discharge and have working relationships with physicians, nurses and discharge planners from all the area hospitals. Anticipating the needs of the most discerning guest, The Avantara Park intersects customized health care with personalized attention.  

Victory Center of South Chicago offers 112 apartments in a supportive living community for adults 65 and older. With a variety of apartment floor plans, Pathway to Living’s award-winning VIVA! programming, 24-hour wellness and care, as well as a host of services and amenities, Supportive Living communities welcome and encourage residents to live life by their own design.

We provide Adult Protective Services though assessments, referrals for in home services, assist with applying for health insurance and advocacy & protection for vulnerable adults.   

Additionally we provide Counseling & Group Support to Caregivers who take care of adults 60 and older or adults 60 and under with Dementia, Alzheimer's Disease, TBI or any kind of neurological disorder.
